    The main objective of this investigation was to identify the business practices in the private offices of physicians in Puerto Rico, and their self perception as small business managers. A questionnaire was created and distributed to a sample of 250 physicians in solo practice from different towns of the island. A descriptive analysis of the results obtained from the survey was performed and found that the majority of doctors in medicine, despite not having received training in office administration, use the business practices of marketing, accounting and human resources management. In addition, the physicians have goals related to quality of service and have some kind of control in the administration of their offices. The majority of the respondents perceived that the external environment influences the internal decisions of their practice, especially the insurance companies. Comparison of business practices according to demographic data revealed that marketing is related to the age and the academic preparation of the health care provider. Moreover, it was found that the recruitment and selection of the office personnel are related to gender and to the place where the degree was obtained. In general terms, the majority of doctors in medicine perceived themselves as small business managers.
